Unsatisfactory Equipment Report

This is the form used to turn in any equipment that is broken or defective.  Does your bayonet not snap into the sheath?  Did the heal of your boot come off?  It’s a pretty cool form, a one-side simple copy on regular white copy paper.  Have fun with the guys at reenactments, make them all fill it out for anything serviceable.  Nothing makes a reenactment more realistic than red tape, right?
